問卷調查是各行各業普遍使用的調查方法，社會科學的學術研究領域尤其常見。問卷調查具有容易普及、擴散的特點，可以同時向多人收集資料。在科技發達、網際網路普及的現在，線上問卷調查系統的出現讓問卷調查更加方便及快速。
社會之所以能夠如此快速的發展進步，仰賴於專家學者不停的創新與探討。社會科學研究領域之學者設計問卷調查，收集人民的心得、想法，並將其應用、實現於社會進步中；相對的，也有自私的專家學者，濫用問卷調查的漏洞，偽造篡改收集而來的問卷結果，並將這些假冒的結果撰寫為學術論文。這些造假的學術論文，影響的不僅是自己的名聲，也影響指導教授、學校，以及每一位讀者，更影響後進學者對於研究誠信的信心。
本研究探討現有之線上問卷調查平台及區塊鏈架構，將兩者結合並提出結合區塊鏈之線上問卷調查系統，將問卷調查的原生資料完整儲存於區塊鏈中。本研究透過改善問卷調查的流程，讓問卷調查取得之結果能夠被有效紀錄及驗證。最後以此系統作為學術研究的把關，讓研究數據增加可信度。

Questionnaire is a survey method that commonly used in all walks of life. Questionnaire is simple to spread and popularize, it also can collect data from multiple people at the same time. Nowadays, Internet and technology are different from the past, online questionnaire platform makes it simpler than before.
Social sciences usually use questionnaire to collect data from people, then analyze the data to make society improving. However, there is some academic fraud. They tamper data to reach their purpose and publish the fraudulent result to public. If these studies were referenced, it may affect the academic integrity of school, advising professor and so on.
In order to prevent academic fraud, this study uses Blockchain. It can propose data to online node and protect it from fraud, also support online questionnaire platform. In this study, storing questionnaire data in Blockchain and protect it from malicious behavior.
