本研究的目的是利用實證方法分析教育與健康行為的關係,我們採用Duan et al.(1983)的二部迴歸模型。本文中的健康行為包含控制體重和健康檢查,不良的健康行為則是吸菸。本文分析教育對這些行為產生正面或負面的影響以及程度的大小,並判斷教育是否直接影響健康行為以及不良的健康行為,或是教育及健康行為同時受到其他因素的影響。此外,我們將進一步分析擁有健康知識是否會影響教育和健康行為;為避免內生性的問題發生,我們使用父母親的教育程度、兄弟姊妹的人數和16歲的居住地等工具變數來預測個人的教育程度。
本研究採用的資料是中央研究院調查的「華人家庭動態資料庫」,根據2004年對主樣本進行的追蹤調查資料,獲取主樣本健康知識以及健康行為的資訊;並連結1999年、2000年以及2003年的資料,獲取工具變數的資料。本研究結果發現教育程度越高者,越重視自己的健康,確實會降低吸菸的比例,減少吸菸量,增加運動量、規律飲食等體重控制的活動,並會多做健康檢查。健康知識對於健康行為是有益的,會減少吸菸的比例,加強控制體重以及增加健康檢查的比例;而且健康知識會取代部分教育的影響力,顯示某部分教育的影響來自於健康知識。根據差異性檢定(Hausman test)的結果,吸菸、體重控制、健康檢查的分析皆採用原來的Probit模型,吸菸量的分析應採用最小平方法(OLS),支持Grossman(1972b)的理論,將教育視為外生變數,認為教育對健康行為的影響至少有一部分是直接的。因此,政府可藉由教育來改善國民的健康行為,加強健康知識的宣導,有效地增進國民健康,進而改善醫療支出浪費的問題。
The main purpose of this essay is to study the relationship between education and health behavior.Four kinds of health behaviors including smoking participation, the amount of cigarette consumption, whether to control weight or not and whether to do health examination or not are included in this study. We also take into account the effect of health knowledge when investigating the impact of education on health behavior. Data used for this study is the Panel Study of Family Dynamics (PSFD) released by Academia Sinica in Taiwan.
An empirical model is constructed to estimate the relationship between education and health behavior. In order to remedy the potential endogeneity problem, we use instrumental variable (IV) approach. In the case of smoking participation, weight control and health examination, a probit model and an instrumental variable (IV) method are employed to test the existence of endogeneity of education variable; in the case of cigarette consumption, we use an ordinary least squares (OLS) method and an instrumental variable (IV) method to test the existence of endogeneity of education variable. Parents’ education level, siblings’ education level and former residence variables are used as instruments for individuals’ education. We find that people with more health knowledge are more likely to adopt healthy behavior. After taking into account the health knowledge effect, education still plays an important role in health behavior. Our estimation results show that there is a statistically negative relationship between education and unhealthy behavior such as smoking participation and cigarette consumption. Also, we find a statistically positive relationship between education and the adoption of healthy behavior such as controlling weight and doing health examination.
